Dr. Sinitsyn: diabetes insipidus life
Diabetics are perhaps the most well-read patients. Too many things to know in order to keep control of this disease. But this all-knowing doctor even diabetics "experienced" listen very carefully, trying not to miss anything. After all, her every word - strong arguments based on the convincing results of the treatment and the tremendous experience of medical practice. "According to doctors at a premium. I am responsible for all the above, I can not just talking the language ", - says Yelena Sinitsyn, head of the endocrinology department of the clinic of the Moscow Institute of Cybernetic Medicine, endocrinologist highest category.

- Elena, you are talking about you as a professional high class. The deserved such a success?

- To be successful in this business, you have to be a fanatic. Here I am in a sense, bigot of his craft and worked always where the conditions provided for my improvement.

I call myself "a soldier of medicine," because it's a serious school, three years in the countryside in the ambulance, and then in the central district hospital, then to the 67-th city hospital, and then I went to the hospital for war veterans - a very good, luxuriously equipped, where she worked for 16 years.

In this hospital, I went deliberately to grow in their profession. There was a hormone laboratory, almost unique at the time in Moscow, and I could use all the features of modern diagnostics, check the level of hormones in the special equipment, and magnetic resonance imaging was then still a novelty. The department where I studied, in its history did not put as much diagnosis of rare diseases, as I put in the hospital, being able to survey people on modern equipment.

The fact that this hospital had such complex patients who because of their age had a whole range of pathologies, allowed me to move forward as a specialist. And in our world of narrow physician if you become an authority, the colleagues always know about it.

- Where did you get interested in diabetes, why him?

- Probably, because the basis of metabolism - the chemistry is my favorite, I always it was easy. At school we had a class with the profound study of the subject. In the first two years of medical school of the future doctor taught chemistry, physics, and everything seems to be that it is so far away from medicine. And I think that the deeper knowledge of these sciences, and all the laws of physiology, the better the doctor. As that's where glucose with insulin interact? The main question diabetology, solid theory. When you know the subject well, you want to better understand more deeply. It draws.

- So you came in endocrinology?

- It was a conscious choice. Working ambulance doctor, I'm interested in cardiology. And there I saw that kardioblok night comes five heart attacks, three of them - diabetics and treat them no one knows, and even the most experienced kardioreanimatologi often lost with a high content of sugar in the blood. It changes the entire treatment algorithm. Decided: it is necessary to become an endocrinologist. It is impossible to treat!

After residency training I was sent to work in the intensive care unit. While in the 67th hospital it was just a very high mortality rate of diabetic com - above 20%. And I lowered it to 6%.

There, in the emergency room, I was cured for the year of about 200 diabetic com. Without boasting, you rarely find an endocrinologist, which many diabetics have brought out of the coma, as I am. After seeing my results, then chief endocrinologist Moscow Alexander Mazowiecki even wanted to open a separate endocrinology resuscitation. But it turned out too expensive to create a separate intensive care for diabetics, and the idea was buried. Diabetics in this sense, no luck ...

- Why?

- Diabetes is not engaged in insurance companies. To insure a diabetic - a huge expense. Insured against such diseases are either cured or require minor time and cost, and diabetes to those not affected.

It turns out, he rushes the patient - depending on whether he is concerned about his health. It is left to itself and can not understand all the intricacies of the disease. And few, very few specialists in endocrinology, which can effectively treat and are not indifferent to the fate of his patient.

Today, my specialty was the leading, because in many respects diabetes - a disease of time. I constantly have to work the brain, and the accumulated knowledge is very relevance. Unfortunately, diabetes can not be treated anywhere, neither in America nor in Europe. There's even a paradoxical situation: states that diabetes is not a disease but a way of life. I have had patients everywhere. I just wonder ineffectiveness of their treatment.

- But how can we assess the effectiveness of therapy known terminal illness?

- By international standards, evaluation criteria - is glycated (sugar-coated) hemoglobin, cholesterol, overweight. In a healthy person the level of glycated hemoglobin in the blood - 6% of diabetics, it can be two times higher depending on the degree of compensation of the disease. Normalize this figure is very difficult. Throughout the world, it is considered a good result to lower its level of at least 1% during the year. In the absolute majority of patients in our clinic it is reduced by 1, 5-2, 7% in the 21 days of treatment.
